Office visits, Mammograms and clinical breast exams, Pap tests, Prostate cancer screening tests recommended by your doctor, Colorectal cancer screening tests recommended by your doctor, Lung cancer screening tests, if recommended by your doctor and you meet criteria, Health education, Help with coordinating associated care,

How to be eligible for this program?

  • You must be a Delaware resident.
  • Have health insurance that doesn't cover screenings.
  • Your age must be 18-64 and not eligible for Medicaid.
  • Your age must be 65 or older and do not qualify for Medicare.
  • Meet income guidelines.
  • Are not eligible for health insurance from the Health Insurance Marketplace.
  • Delaware Residents are eligible for Screening for Life if they have a household income between 139% and 250% of the Federal Poverty Level.
  • The Federal Poverty Level is based on annual household income and household size.
  • A household consists of the individual, their spouse, and all dependent children under 18 years of age.
  • Delaware Residents with an annual household income between 139% and 250% of the Federal Poverty Level are eligible for Screening for Life if they are deemed ineligible for Medicaid.

What are their Insurance Guidelines?

  • Uninsured: No medical insurance nor private HMO, Not eligible for Medicare or Medicaid, Not eligible for health insurance through the Health Insurance Marketplace Underinsured: Unmet deductible (deductible must be more than 15% of annual income), Insurance does not cover breast, cervical, colorectal, lung, and prostate screenings.
  • he SFL program does not reimburse for co-pays or co-insurances.
