Benefits
Clinical breast exams every other year performed by a nurse or physician for women ages 40 to 64
Screening mammograms every other year for women at average risk age 40-64.
Limited services are available for uninsured women at high risk under age 40; for information call the toll-free number 1-877-277-1368.
Eligibility
Women 21 to 64 years old for cervical cancer screenings.
Women 40 to 64 years old for breast cancer screenings.
Women who do not have health insurance.
Women with family income or equal to or less than the amounts on the following table, before taxes:

Enrollment takes five to ten minutes. To start, call 877-277-1368.
If you qualify you will receive an enrollment number and be referred to a health care provider. Your enrollment number is good for six months.
Keep it in a safe place and remember to share it with your provider at the time you make your appointment.
