Eligibility
Have no private health insurance, Medicare, Title 19, or Medicaid (except limited benefits under the Family Waiver Plan), or CDIB.Have income that does not exceed 185% of Federal Poverty Guidelines.
Have a clinical breast exam at a participating clinic or by your private physician within two months of the mammogram.

After receiving your OPW coupon, call any of the participating institutions below to schedule your SCREENING or DIAGNOSTIC mammogram or ultrasound appointment. Please note: All facilities do screening mammograms but not all facilities do diagnostic mammograms. You will need a doctor’s order/prescription for any diagnostic testing. Tell the scheduler you have an Oklahoma Project Woman coupon when you make the appointment. You MUST bring the COMPLETED AND SIGNED coupon with you. It pays for your mammogram and/or ultrasound.Mission
Oklahoma Project Woman is dedicated to providing access to breast health care that will facilitate the early diagnosis of breast cancer and decrease the mortality rate for uninsured women and men with limited income.
